Before your first appointment…

1. Check your insurance coverage

Call the member services number on the back of your insurance card and ask the following:

Does my plan cover outpatient nutrition counseling?

  • If yes, how many visits are allowed?

  • Is coverage based on medical necessity? If so, are there certain diagnoses covered/not covered for counseling?

Do I need a physician referral?

  • If yes, please see referral information on step 2 below.

How much will my co-pay be for outpatient nutrition counseling?

  • Do I have a deductible to meet before co-pay applies?

We currently accept BlueCross BlueShield, Harvard Pilgrim, Aetna, Mass General Brigham Health Plan (formerly AllWays) and United Health Care. If we are out of network, a superbill may be provided.

It is the patient’s responsibility to inquire about insurer coverage and attain accurate provider information regarding coverage.


2. Obtain a referral

If your insurance plan requires a referral for nutrition counseling, please contact your physician’s office at least one week prior to your initial appointment. We have provided information they may request below. If additional information is needed please contact us.

PLEASE NOTE~ if you have a Harvard Pilgrim plan, they require a physician’s order for nutrition counseling services. Please have your physician complete the physician order form before your first appointment.

3. Register for patient portal
